The details are below:\n\nAdditional Details\n- Is your feedback related to a site?: No\n- Additional detail: I followed the process to update my ID card as described here Photo ID application form | Corporate Facilities | Department of Health. I was advised via email that the process would take about 10 business days and that I would be notified when my new card was ready to be picked up.\n\nAs you can see below, I did not receive my card. I did not receive any communication, even after two follow ups. Today, I called the 1800 number because my card will expire tomorrow. I was advised (rudely) that my new card is sitting at the security desk. Obviously something in the process is broken, not only because I was not notified that my card was ready, but also because I got no replies to two emails to the forms@qh.digital account.",
